Is this a hardware problem?

I have a recent MacBook Pro. After frequent crashes of Safari and other apps and failure to wake from sleep, I checked the hard drive with Disk Utility. I got this:

Checking Catalog file.
Invalid extent entry
Incorrect block count for file PageCapThumbV2-5.tiff
(It should be 0 instead of 33554434)
Keys out of order
The volume Macintosh HD needs to be repaired.
Error: The underlying task reported failure on exit

Volume needs repair
When I repair the disk from a startup disk it says "Repairing disk failed with error Could not unmount disk"

This is the third time this has happen. Does the above suggest a hard drive failure of the drive? If so, is there a utility I can download to confirm that my drive has a hardware failure?

Followup: I can unmount the disk but I still get recurring errors despite Repairing the Disk. Thanks again for any help!

Indeed, I can't even repair the disk. After the fourth repair attempt I get:

Verify and Repair disk “Macintosh HD”
Checking HFS Plus volume.
Checking Extents Overflow file.
Checking Catalog file.
Invalid extent entry
Incorrect block count for file PageCapThumbV2-5.tiff
(It should be 0 instead of 33554434)
Keys out of order
Rebuilding Catalog B-tree.
The volume Macintosh HD could not be repaired.

Error: The underlying task reported failure on exit


1 HFS volume checked
1 volume could not be repaired because of an error
Repair attempted on 1 volume
1 volume could not be repaired


Do people recommend reinstalling 10.4.x or scheduling a hard disk repair?

Back it up ASAP if you can because that drive is dying/dead. Replacing the drive is most likely in your future.

In these situations boot from the installation cd/dvd. Than the unmounting will not be a problem. Check this first before making any definite conclusions.

If the volumes keeps getting damaged, most likely the drive is bad, but other hardware (like bad memory) can also cause volume errors to occur.

DiskWarrior can usually fix this error. If you have the latest version, give it a try.
