iTunes crashing since install of Facebook MyMusic

Help! My iTunes has been crashing repeteadly with this msg:



Since I installed this crap (it's the leftmost icon):



It was installed through one of the utilities at Facebook supposedly allowing you to post your iTunes library on Facebook.

I've removed the offending application on Facebook, but it's still in my system!

Unfortunately, I can't find it in the apps folder and running the system monitor and looking at all tasks did not result in my finding the offending task and file. Any advice on how to uninstall it would be greatly appreciated!!
