I should probably add that the problems occur at random times, and do not appear to be specific to any applications. I've tried various things to troubleshoot. Thinking that it was a problem with my original G4 USB ports, I installed a PCI USB card, but am having the issues. I've run Disk Utility, Software Update, zapped my PRAM, rebooted in safe mode and defragmented fragmented files (not necessarily in that order). I am unable to run Apple Hardware Test (from the CD that came with my computer) for some reason.

The information in the panic log points to a problem with USB. If you are using a third-party mouse and/or third-party mouse software drivers I would look there first by removing the third-party software. If the problem persists, then replace the mouse with the original Apple mouse. If it isn't a mouse related problem, then it's some other USB device or a hardware problem.

You could also try erasing the hard drive and reinstalling OS X. Be sure to backup your current system as a precaution.

If none of the above fixes the problem, then you will need to take the computer to an Apple service center.

I'll take your advise and try erasing the hard drive and reinstalling OSX. My G4 (Digital Audio) is a few years old, and I have installed several versions of the OS over the years and am currently running OS 10.3.9...what is the best way to go about backing up my data and reinstalling?

There are many ways to deal with backup and restore. Check out http://www.thexlab.com/faqs/faqs.html and read the FAQ on backup and restore. Here's another good article on the topic: http://docs.info.apple.com/article.html?artnum=106941.

There are a lot of excellent backup utilities available such as Carbon Copy Cloner, Super Duper, LaCie SilverKeeper, Retrospect, Deja Vu, Synchronize! Pro X, and for basic full backup you can use the Restore option of Disk Utility.

When I posted the kernel panic log above, I neglected to hide my personal info, including things like my IP address. As a result, this page appeared when I did a Google search on the name "Hilo Osawa". I'm concerned about my privacy...is there a way of deleting any personal info from my post above? Please advise as soon as you can. Thanks in advance!

Your IP address is known to every single website you visit, and they keep logs of that information that are available to Homeland Security, CIA, etc. so I wouldn't worry too much about the IP address. Reviewing your info, the only mistake you made was putting in the serial number of your Macintosh. You realize, however, that this page has been cached my a multitude of search engines and deleting the serial number now, almost a year after you posted it will not help. You might want to call Apple, though so that they can flag it so that in the remote chance that someone tries to use your serial number for an unauthorized repair on your computer, they will know it's not you and you won't be billed for the repair. Honestly, though, I don't think you have much to worry about.
