In Verbose mode we get a kernel panic and see the message: failed to load extension com.apple.driver.appleusbhub

This started with a upgrade from Panther 10.3.9 to Tiger 10.4 then a incrimental "Software Update" to 10.4.9 by an unskilled computer user who asked me to troubleshoot this new problem. Now we cannot get past the black screen of death: "You need to restart your computer. Hold down the Power button for several seconds or press the Restart button" when starting normally. If I start in verbose mode (apple V) I get a kernel panic and I see the message: failed to load extension com.apple.driver.appleusbhub in yellow scrolling by quickly. I am able to use DiskWarrior for Tiger and rebuild the directory, but nothing else works. Any suggestions?

I can start into Open Firmware (Apple, Option, O, F) successfully and have reset NVRAM with no success.
Safe boot (holding down shift key while starting up) gets the message "You need to restart your computer. Hold down the Power button for several seconds or press the Restart button"

Kevin,
Did you try to start up the computer using the 10.4 full install disk?
Unplug all the devices except the keyboard and mouse. Then insert the 10.4 install disk. Restart the computer and immediately hold down the C key.
You should be able to do an archive and preserve install.
Please be aware that you can not use an upgrade disk or a disk specific to a different model computer. You can use the 10.4 install, then upgrade to 10.4.9 using the combo update that Software Update recommends.
Let me know if you need step by step instructions or if you have already tried this.

Reinstalling 10.4 after complete erase of HD, will let you know the outcome. Will install the combo updater 10.4.9 next.

Seems to be working after installing the combo updater 10.4.9. Thanks for your help!
