Mac Help does not work after reinstallation OS X.4 in iMac G5
Others Help work (Adress book, airport ...)
What can i do?

Try the following:
In most cases doing the following will restore the Help Viewer:
1. Trash the following three files, if extant:
/Home/Library/Preferences/com.apple.help.plist
/Home/Library/Preferences/com.apple.helpui.plist
/Home/Library/Preferences/com.apple.helpviewer.plist
2. Trash the /Home/Library/Caches/com.apple.helpui folder.
3. Empty the Trash and Restart.

Thanks, but works only once.
MacHelp works if i trash /Home/Library/Caches/com.apple.helpui folder and disconnect internet, without restart. Then works quickly. If internet connected Mac Help does not work more and i have to trash again the folder.

Did you also delete the .plist files or just the cache folder? Did you restart?

I trahed first all files indicated. Did not work, only opened once then blank window.
After that i trashed only the /Home/Library/Caches/com.apple.helpui folder; works good without restart if Internet is not connected (con Cable modem or con phone modem). If i open Mac Help with Internet connected I have to trash again the folder.

Hmmm, well that's truly odd. Open the Accounts pref pane and see if you have an entry, iTunesHelper. If so delete the entry. Delete the help cache folder and .plist files again, then restart. See if that helps. Also, because I don't know the actual version of OS X you are running, at least be sure to upgrade Tiger to 10.4.3.
