I took out the internal modem and connected a Zoom 56kx
modem. I am having problems staying connected, especially
when I use Apple's Internet Dialer. Are there any extensions that I should disconnect?

You have to remove the internal modem software.
Remove the following if you have them.

Extensions
Apple Telecom Modem
Express Modem Tool
GeoPort for Power Macintosh
GeoPort Serial Driver
GeoPort Telecom Adapter
GeoPort Telephone Tool
GeoPort/Express Modem
GeoPort/Express Modem CNG
Preferences
Express Modem Preferences
