You can change whether iPhoto gets to import from a camera in the app "Image Capture", located in your applications folder. Select preferences from the app menu to change settings.

I believe you should have a Nikon Twain plug-in in your_username/Library/Image Capture/TWAIN Data Sources/ Is it there?

Try using Image Capture with the scanner on and plugged in to the computer, see if that detects and scans. If that doesn't work I have had good luck with VueScan look for it on Google. ...

Image Capture, being an Apple app, is creating the preview and icon views on the fly as it saves them. You can be sure the camera is not creating them. Why those images don't have the icon views I can't guess. If you experiment you'll probably figure it out.
